The purpose of this memorandum is to transmit a print-ready, master copy of all ADE required special education forms for use in implementing all provisions of the IDEA. The print-ready, master copy will be sent via U.S. mail. The print-ready set of forms is being provided now to allow you ample time to reproduce the forms prior to the opening of the 2000-2001 school year. You are to destroy any copies of forms that you have previously used. The effective date for the use of these forms is JULY 1, 2000.
You are being provided a print-ready copy of "Your Rights Under the IDEA" also. The content has not changed, only the title, consequently you may continue to use copies of the previous document, "Parents' Rights Under the IDEA" until you have exhausted your current supply. Upon reprinting, you are to use the re-titled version.
The following forms are being provided:
A. Due Process Forms applicable to children with disabilities, ages 3 - 21 (including "Your Rights Under the IDEA")
B. Due Process Forms applicable ONLY to children with disabilities,
ages 3 - 5
C. Versions A and B of the Individualized Education Program (IEP). [Only one version is to be used with an individual student. If a teacher is using the computerized IEP, he/she will not use a print version from this packet. Note: The transition section is not applicable to students under the age of 14.]
D. Special Use Forms including --
· Due Process Hearing/Expedited Hearing
· Functional Behavioral Assessment (FBA)
· Manifestation Determination Review (MDR)
· Notice Regarding Issues in Parental Private School Placement
· Surrogate Parents (SP)
· Transfer of Rights
As you can see, the majority of these forms may never be needed. However, it is crucial that you have a complete master set available for printing those forms that are needed by the district. The forms may also be accessed on the ADE web site at http://arkedu.state.ar.us from Appendix A of the document Special Education and Related Services: Procedural Requirements and Program Standards (ADE, 2000.).
