The Individuals with Disabilities Education Act Amendments of 1997 require that specific disciplinary information be submitted to the U.S. Department of Education as an annual data reporting requirement. The forms and instructions for completing the 2000-01 school year reports are attached.

This report consists of two categories. In Section A, the discipline data are reported by disability category. In Section B, the same data are reported by ethnicity.

ELECTRONIC SUBMISSION OF THE DISCIPLINARY ACTIONS DATA REPORT FOR DISABLED STUDENTS:

The report of specific disciplinary information on students with disabilities will be submitted electronically through Cycle 7 of the Statewide Information System. The instructions for Cycle 7 will be on the APSCN web page at http://www.k12.ar.us/apscndocs/sis.htm. Electronic submission of these reports is required of all school districts on the APSCN system.

Completed reports are due June 15, 2001.
