Attached is the fiscal year (FY) 2002 application for Extended School Year Services. For instructions, please refer to the attachment.
The application for extended school year services consists of two parts. This is necessary because the period of time during which extended school year services may be provided transcends two fiscal years.
Part One covers the period of time from the close of the current school year through June 30. Applications are due in the Special Education Office by May 31, 2001.
Part Two covers the period from July 1 through August 31. Applications are due in the Special Education Office by September 14, 2001. Payments will be made in October.
Reimbursement:
The reimbursement for the period ending June 30, 2001 (Part One) will be $20 per student per day. Pending approval by the State Board of Education, the reimbursement rate for the period beginning July 1, 2001, through August 31, 2001, (Part Two) will be $20 per student per day.
NOTE: Because of the limited amount of funds available, it is very important that all reimbursement requests are submitted BEFORE each deadline. Requests received after a reasonable amount of time will not be reimbursed.
