11) Temporary Placement Consent
12) Consent for Initial Placement
13) Parental Notice of Conference Decision
14) Notice of Transfer of Parental Rights (Parents)
15) Notice of Transfer of Parental Rights (Student)
When these materials are received, please provide copies of these print-ready materials to all building principals/designees responsible for special education due process.
