School districts are required to report annually the enrollment and number of children eligible for Free and Reduced price meals as of October 1. This information will be required in order to process a school district claim for reimbursement for the month of October, 2001.
I. Student Eligibility Data for EACH SCHOOL
A copy of the APSCN Cycle 2 Child Nutrition Report (Number SIS: 431) will be submitted for each school. For every child coded as eligible for Free or Reduced price meals there must be a currently approved application on file or his/her name must be included on the direct certification list for free students. These source documents must be retrievable by school regardless of the school district's method for certifying eligibility whether it is centralized or by school.
II. Total Student Eligibility Data for the DISTRICT
A copy of the APSCN Cycle 2 Child Nutrition Report (Number SIS: 331) will be submitted for the district. District eligibility totals of Free, Reduced and Paid must balance with the total of all SIS: 431 reports by category and be supported by the source documents listed above.
The total number of students eligible for Free and Reduced price meals should be very close to the total number reported for the lunch program on line 1F of the October claim. The claim for reimbursement requests the highest numbers of students eligible for free and reduced price meals served on any day in October, which could be slightly different from the numbers on October 1, 2001.
III. Student Eligibility Data for (1) Pre-K, (2) Students in Adult Education Centers, (3) Students from Outside District- COMPLETE ATTACHED FORM
Since APSCN Number SIS: 431 Reports do not include children participating in (1) Pre-K, (2) Adult Education Center Students or (3) Students served who are enrolled in other districts, the attached 2001-2002 Eligibility Report must be completed for any other district students not included in the APSCN SIS: 431 Report.
IV. Submission of all data to the Child Nutrition Unit, ADE
Return the following reports ATTACHED TO the district's October Claim for Reimbursement:
A. School Data - Copies of the SIS: 431 Report for each school
B. District Data - Copy of the SIS: 331 Report for the district
C. 2001-02 Eligibility Report signed by the superintendent
The October Claim for Reimbursement will not be processed without the complete APSCN Cycle 2 Child Nutrition Report and 2001-2002 Eligibility Report.
