The attached forms and instructions are provided for your use in submitting your December 1, 2001 Child Count. PLEASE NOTE THAT DECEMBER 1 FALLS ON A SATURDAY THIS YEAR. AS A RESULT, THE ACTUAL COUNT DATE IS DECEMBER 3, 2001.

Please note that the reports include necessary information regarding age, sex, ethnic designation, primary disability category, and educational placement. Data submitted will be used for the Annual Data Report required under IDEA and other necessary information concerning children with disabilities.

In order for a district to receive federal funding for students with disabilities, a student must be:

1. identified to be in need of special education services in accordance with state and federal regulations, and

2. have an individualized education program in place and be receiving services on December 1 (December 3).

Electronic Submission of the December 1, 2001 School Age Child Count:

The December 1, 2001 School Age Child Count will be submitted electronically through Cycle 4 of the Statewide Information System. The instructions for Cycle 4 will be on the APSCN web page at http://www.k12.ar.us/apscndocs/sis.htm. ELECTRONIC SUBMISSION OF THE DECEMBER 1, 2001 CHILD COUNT IS REQUIRED OF ALL SCHOOL DISTRICTS ON THE APSCN SYSTEM. For districts on the APSCN system, the attached forms are for informational purposes only. DO NOT send a copy of the count to the Special Education Unit. You will not be required to submit the certification page (page 27).

Deadline for submitting the December 1 count is December 15, 2001. Failure to submit an official count form will result in no Part B funds for FY 2003.

NOTE: Before submitting the count to APSCN, please review thoroughly to ensure that the report is accurate. Compare the 2001 count to the 2000 reported count and check for any significant changes. If errors are discovered after the report has been submitted, REVISED REPORTS INCREASING OR DECREASING THE COUNT WILL BE ACCEPTED THROUGH DECEMBER 31, 2001. THEREAFTER, ONLY DOWNWARD REVISIONS WILL BE ACCEPTED. Please send your revisions to Mr. Clent Holly at the following address:
