Each year the Arkansas Department of Education makes available Form FAPD-2 (Resident and Non-Resident Quarterly Report of Enrollment and Attendance), report forms and Attendance Registers - all of which are optional for use in the schools. These supplies are ordered and distributed according to information provided to us from each school district. Since these supplies are ordered only one time a year, please make sure requests for these forms meet your needs for the year.
To ensure accurate printing and prompt delivery of these forms, please return the attached form by February 19, 2002.
