It is time once again, for the Department of Information Systems (DIS) to file an E-rate Form 486 on behalf of all Arkansas public school districts and cooperatives for Funding Year 2002 for the delivery of Internet services and the APSCN network. In order to file Form 486 with the Schools and Libraries Division, DIS must first receive from each public school district and cooperative a Form 479 to certify their compliance with the Children's Internet Protection Act. If DIS is unable to submit Form 486 to the SLD in a timely manner, due to districts not submitting Form 479, DIS will lose discounts from the E-rate program, and be forced to pass the costs back onto the districts.
Please do not confuse this submission with Form 479 that was submitted during the Fall of 2001 for E-rate Funding Year 2001. Every district and cooperative is required to submit a new Form 479 for Funding Year 2002.
