See a list of districts eligible for these rates.
Severe Need Breakfast
Severe Need Breakfast rates are approved on a school by school basis after application is made by the school district. Contact Jimmy Burks at (501) 324-9502 for additional information.
CURRENT FORMS:
Enclosed is a copy of the following report forms. Please make additional copies as needed.
Claim for Reimbursement - The revised Claim Form has the new reimbursement rates for 2002-2003. PLEASE DISCARD THE OLD CLAIM FORMS. This form is available on Excel. To request an Excel copy, please e-mail Beverly Christenberry. Only one request per school district should be submitted.
This form prints best on legal size paper. Instructions for completing the Claim for Reimbursement are included on back of form.
Daily Record Form - It is recommended that a separate record be maintained for each school as listed on Schedule A of the Agreement. Participation figures, by school, are required for severe need breakfast reimbursement. If you do not apply for severe need breakfast reimbursement and wish to keep only one record for all students served in the same location (even if this includes more than one school), you may do so. If this is done, all schools combined on one report will be reviewed when state or federal reviews are made. This form may be modified if a local district needs additional information. The cash management worksheet on the back will provide good management information. This form is available on Excel. To request copy, please e-mail Beverly Christenberry.
Edit Check Worksheet - This is the current form recommended for edit checks required by regulation. It may be modified to meet local needs; however, the comparisons required on this form must be a part of any form used.
On-Site Review Form - Regulations require that each district (with more than one serving site) conduct on-site reviews of each school by February 1 of each year. It may be modified to meet local needs; however, the comparisons required on this form must be a part of any form used.
Verification Summary - This is the current form recommended for the summary of verification results. It may be modified if you choose; however, the information required on this form must be a part of any form used. The verification process must be completed by December 15, including the completion of a verification summary.
