The Department of Information Systems (DIS) will be filing E-rate Form 486 on behalf of the Arkansas Department of Education (ADE) for all Arkansas public school districts, charter schools, and education cooperatives for Funding Year 2003 for the delivery of Internet services and the APSCN network. Before DIS can file Form 486 with the Schools and Libraries Division, the ADE must receive FCC Form 479 from each public school district, charter school, and education cooperative. Form 479 is used to certify that each entity is in compliance with the Children's Internet Protection Act. If districts do not submit Form 479 to the ADE before DIS must file Form 486, DIS will lose discounts from the E-rate program and be forced to pass the costs back onto the districts. Forms 479 and 486 must be submitted each funding year.
We are now requesting that each district, charter school, and education cooperative complete Form 479 for Funding Year 2003 and submit it to the ADE. For your convenience, Form 479 and instructions for completing the form have been attached to this memo. Both documents are in .pdf format which can be downloaded and printed. You can also download the form and instructions from the Schools and Libraries Division web site: http://www.sl.universalservice.org/form/.
