The information in IA-04-058, dated 12/19/2003, is exactly the same as in IA-04-017, dated 08/07/2003; only the number and date are changed.
The Arkansas Department of Health has finalized the Rules Pertaining to Immunization Requirements. The revised rules contain the changes with regard to the procedures for parents and/or guardians applying for the religious or philosophical exemption.
Replace previous versions of the immunization regulations with this revision which has an effective date of August 15, 2003.
An electronic version of the rules is not available at this time, therefore a paper copy will be placed in mass mail.
