SCHOOL DISTRICT PROCEDURES FOR SUBMITTING NOTICE OF INTENT TO HOME SCHOOL FORMS AND CONFIDENTIALITY OF INFORMATION ON INTENT AND WAIVER FORMS:

SUBMIT TO DEPARTMENT OF EDUCATION (ADE): Submit a legible copy of each Notice of Intent to Home School form (front and back) filed with the local school district. Continue to submit the Intents to the Home School Office, during the school year, as received from parents/legal guardians.

SUBMIT TO COOPERATIVES: Submit a LEGIBLE copy of the front of each Notice of Intent to Home School form that has a student listed in grades 5, 7, or 10. (Do not send copies to cooperative unless a student is listed in grades 5, 7, or 10 on the Intent.) This is to facilitate the required home school testing process for the 2003-2004 school year. The cooperatives will need legible information in order to inform home school parents of the testing dates, times and locations. This information is also necessary for cooperatives to secure an adequate number of test administrators. Districts should submit forms (grades 5, 7, and 10) to cooperative as soon as received from parents.

After the testing window is complete, districts no longer need to submit copies to the cooperatives. Do not send copies of the Waiver form. If districts have further questions concerning home school testing, contact the director of the applicable cooperative. The three school districts in Pulaski County conduct testing for the local district.

**** Remind the persons processing the Notice of Intent to Home School and Waiver forms that the information contained on the forms may be used only for statistical purposes and test administration. IT IS ILLEGAL TO RELEASE OR USE THIS INFORMATION FOR ANY OTHER PURPOSE.

**** The home school law does NOT require that parents/legal guardians be interviewed prior to submitting the Notice of Intent to Home School and Waiver forms to the superintendent’s office.

School districts should submit Notice of Intent to Home School forms to:
