Beginning July 1, 2004, all school districts submitting waiver requests to meet requirements of Act 1623 for those teachers employed out-of-area/grade level longer than thirty (30) consecutive days, must also include a completed copy of the Additional Licensure Plan (ALP) for the out-of-area/grade level assignment.
Waiver request will not be considered for approval, without a copy of the Additional Licensure Plan form being submitted with the request.
