School districts are required to report annually the enrollment and number of children eligible for free and reduced-price meals as recorded in the APSCN Cycle 2 Report. The following ATTACHMENTS TO THE school district's SEPTEMBER 2004 Claim for Reimbursement are REQUIRED:

· K-12 APSCN Cycle 2 Reports SIS Report rpt431 and rpt331
· Pre-School Child Nutrition Reports SIS rpt436 and SIS rpt336
· “Other Students 2004-05 Eligibility Data Report ”

There are THREE MAJOR CHANGES in the process:

· THE DEADLINE FOR THE SEPTEMBER CLAIM is extended to OCTOBER 15, 2004, to coincide with the deadline for the APSCN CYCLE 2 REPORT.
· Please note all reports must be attached to the September claim NOT the October claim. This change is necessary to more quickly verify the number of eligibles for determining current year National School Lunch Act (NLSA) funding.
· Pre-Kindergarten students who eat in the National School Lunch Program operated by the school district will be reported on the new Cycle 2 October 1 Report rpt436 and rpt336.

All SCHOOL REPORTS for rpt431and rpt436, and all DISTRICT REPORTS FOR rpt331 and rpt336:

· MUST AGREE WITH CHILD NUTRITION DOCUMENTATION of eligible Free and Reduced-Price Students BEFORE SUBMISSION to APSCN and the Child Nutrition Unit, Arkansas Department of Education (ADE).
· Reports must be printed on the SAME DATE AS THE FINAL RECONCILIATION of the APSCN computer system count of free and reduced-price students and with child nutrition physical count of documented free and reduced-price students. Computer data must match source documents.

DESCRIPTION OF REQUIRED REPORTS:

1. APSCN CYCLE 2 REPORT SIS rpt431 (K-12) and rpt436 (Pre-K) “ Student Eligibility Data for EACH SCHOOL”

ATTACH COPIES OF EACH SCHOOL’S SIS rpt431 and rpt436 REPORTS TO THE SEPTEMBER CLAIM FOR REIMBURSEMENT. For each student coded as eligible for free or reduced price meals there must be a currently approved application on file OR his/her name must be included on either the direct certification list OR the list provided by the homeless liaison OR the list provided by the migrant coordinator. These source documents must be retrievable by school regardless of the school district’s method for certifying eligibility, whether it is centralized or by school.

Provision 2 schools will follow a completely different procedure and should contact the local Child Nutrition area specialist.

REMEMBER THE STUDENTS CODED AS ELIGIBLE FOR FREE AND REDUCED-PRICE MEALS ON APSCN PRINTOUT MUST MATCH THE CURRENT LIST OF FREE AND REDUCED-PRICE ELIGIBLES IN THE CHILD NUTRITION DEPARTMENT BEFORE THE APSCN REPORT IS SUBMITTED.

2. APSCN CYCLE 2 REPORT SIS rpt331 (K-12) and rpt336 (Pre-K) “Total Student Eligibility Data for DISTRICT “

ATTACH COPIES OF THE DISTRICT’S SIS rpt331 and rpt336 REPORTS TO THE SEPTEMBER CLAIM. DISTRICT eligibility TOTALS of free, reduced and paid students must balance with the TOTAL of ALL SCHOOL SIS rpt431 (K-12) REPORTS and ALL SCHOOL rpt436 (PRE-K) REPORTS by category and be supported by the source documents listed above.

3. OTHER STUDENT ELIGIBILITY DATA for (1) Adult Education Center Students or (2) Students from Outside the District MUST BE REPORTED. COMPLETE ATTACHED FORM EITHER to report data as requested OR to verify the lack of OTHER STUDENTS in the district.

Since APSCN SIS rpt431 or rpt436 School Reports DO NOT include OTHER STUDENTS participating in (1) Adult Education or (2) Students served REIMBURSABLE MEALS and enrolled in other districts, this separate report must be completed. If the district has no OTHER Students, COMPLETE THIS REPORT CHECKING” NOT APPLICABLE” BOXES. TO BE PROCESSED, ALL September claims must have this report attached. The SUPERINTENDENT’S SIGNATURE MUST BE ON THIS REPORT TO BE ACCEPTED.

Attach the following SIX items together and fax or mail to the Child Nutrition Unit, Arkansas Department of Education (ADE):

1. SEPTEMBER Claim for Reimbursement
2. SCHOOL DATA– Signed Copies of the SIS rpt431 (one for each school)
3. SCHOOL DATA – Signed Copies of the SIS rpt436 (one for each pre-school)
4. DISTRICT DATA – Signed Copy of the SIS rpt331 (K-12)
5. DISTRICT DATA – Signed Copy and SIS rpt336 (Pre-K)
6. OTHER STUDENTS 2004-05 ELIGIBILITY REPORT signed by the Superintendent.

THE SEPTEMBER CLAIM FOR REIMBURSEMENT WILL NOT BE PAID WITHOUT THE COMPLETE APSCN CYCLE 2 CHILD NUTRITION REPORTS AND OTHER STUDENTS 2004-2005 ELIGIBILITY REPORT.
