Please save this document to the computer of the person responsible for completing the Schedule of Payments. After entering data to the form, save the completed form. Attach the completed form to an e-mail to email@example.com.
In the 2005-2006 school year, the Excel forms provided must be used. The schedule of payments must be e-mailed. A confirmation that the report has been received in readable form will be provided. If there are problems with the form, the district person transmitting the report will be notified by e-mail. No handwritten, typed, or faxed schedule of payments will be accepted.
