The Department of Information Systems (DIS) will be filing E-rate Form 486 on behalf of the Arkansas Department of Education (ADE) for all Arkansas public school districts, charter schools, and education cooperatives for Funding Year 2007 for the delivery of Internet services, video and APSCN networks. ADE must receive FCC Form 479 before DIS can file Form 486 with the Schools and Libraries Division.
Form 479 is used to certify that each entity is in compliance with the Children’s Protection Act. It is imperative that the information requested on the form be provided. If not, DIS will be unable to complete the FCC Form 486. This will in term result in DIS losing discounts from the E-rate program, which will result in DIS forcing the costs back to districts.
Form 479 for Funding Year 2007 should be completed and faxed by Friday, June 8, 2007 to: Wilma Toombs at 501-371-1942. Upon receiving Form 479 a receipt of acknowledgment will be emailed to the contact person listed on the form. For handwritten submissions use a black pen and clearly print in the spaces provided.
