If a school district selects new services from the 2008-09 E-Rate eligible services list that were not included in the district's current approved technology plan, the school district is required to submit an addendum to the current approved local district technology plan in order to comply with E-Rate application requirements. The revisions should be submitted to the Department of Education by Monday, December 3, 2007. Each plan will be reviewed for approval based on the criteria and standards outlined in the attached documents. The Department will provide a certificate of approval to those districts who have met all the criteria of the technology plan.

A valuable resource for school district technology planning is the 2008 Eligibility Service List (ESL) which is available at the Universal Service Administration Company (USAC) website http://www.usac.org/sl/.

Purposes of Request for Addendum to Approved Local District Technology Plans:
1. To help Arkansas schools become eligible for E-Rate discounts. The FCC stipulated that requests for E-Rate discounts must be based on an approved technology plan (FCC Order 97-157, Paragraph 573).
2. To make districts eligible for other state and federal technology funds.

Submission Deadline: All plan revisions must be received in the Deparment of Education by the close of business (4:00 p.m.) on Monday, December 3, 2007. No faxes or emails will be accepted.

Reminder:
1. The current approved technology plan must include all eligible services planned for Form 470 (year 2008-2009) to be in USAC compliance.
2. A revised budget must be submitted even if there are no changes in the addendum. This is needed for the filing of the E-Rate state application for services such as DIS, APSCN, CIV lines. Please remember that districts can only apply for E-Rate services that are requested on the Form 470 application and included in the approved technology plan.
