The Arkansas Department of Education (ADE) is requesting that an addendum to the local district technology plan be submitted. This addendum is needed:
1. To help Arkansas schools become eligible for E-Rate discounts. The Federal Communications Commission (FCC) stipulated that requests for E-Rate discounts must be based on an approved technology plan (FCC Order 97-157, Paragraph 573). The current approved technology plan (2009-2012) must include all eligible services planned for Form 470 (year 2009-2010) to be in USAC compliance.
2. To make districts eligible for other state and federal technology funds.
A school district that selects new services from the 2009-2010 E-Rate Eligibility Service List (ESL) that were not included in the district's current approved technology plan is required to submit an addendum in order to comply with E-Rate application requirements. The ADE will provide a certificate of approval to the districts that have met all the criteria of the technology plan addendum.

Submission Deadline: All addenda must be received by 4:00 p.m. on Friday, December 12, 2008. No faxes or emails will be accepted.
