The Arkansas Department of Education (ADE) is involved in a nationwide project to examine teacher evaluation and teacher effectiveness. To this end, some self-examination is necessary. All school districts are hereby asked to submit to Ms. Barbara Culpepper, by mail or fax, a copy of the standard Teacher Evaluation Form in use by the school district. Indicate the district name and the name of a contact person on the submission. Please comply with this request by Friday, April 23, 2009. The address and fax number are indicated below.
