The Professional Licensure Standards Board (PLSB)has revised the allegation form to be used to file an allegation of perceived violation of the Code of Ethics for Arkansas Educators.

The revised Allegation of Violation Form may be found at ArkansasEd.org by clicking on Teachers in the green bar at the top of the page, then click on the fourth bullet - Professional Licensure Standards Board, scroll down and click on the fourth bullet or click on this hyperlink: http://www.arkansased.org/standards_board/word/allegation_form_101408.doc The form may then be printed for completion and submission.

Completed forms may be submitted to the PLSB through the Arkansas Department of Education (ADE) (Assistant Commissioner for HR/Licensure), local school board, the Arkansas State Board of Education (ASBE), or public school superintendent.
