Food Service Management Companies (FSMC) that wish to contract for food services with any School Food Authority (SFA)/Local Education Agency (LEA) in Arkansas will be required to register with the Arkansas Department of Education (ADE), Child Nutrition Unit (CNU), no later than 4:30 p.m. on October 30, 2009. Only those Food Service Management Companies registered with ADE CNU by October 30, 2009, will be eligible to respond to the Request for Proposal (RFP) released by Local Education Agencies for the school year 2010-2011. Registrations received after the deadline will not be considered.
LEAs will be notified of the Official ADE List of Registered FSMCs at the time the LEA receives written approval by ADE CNU of the LEA completed Prototype Request for Proposal for FSMC Services. LEAs must secure in writing the approval of the completed RFP FSMC Prototype PRIOR to release for bidder’s responses.
In addition to the FSMC Vendor Registration Form, Food Service Management Companies must submit the attached Certification of Debarment and Suspension Form (7CFR Part 3017.300), Lobbying Certification Form, and Disclosure of Lobbying Activities Form. Submission of the Certification of Debarment and Suspension Form for the registration process does NOT relieve the FSMC of its responsibility to submit a CURRENT Certification of Debarment and Suspension Form to each SFA/LEA as part of the LEA completed RFP document that is submitted for state agency approval by the LEA.
