This is a reminder that any changes in the district’s configuration for the 2010-2011 school year must be sent in on the LEA Number Assignment/Change Form BEFORE August 1, 2010.
A form is required if there is change in the LEA configuration for any of the following:
School name change
Grade span change
An LEA is inactivated (building is closed)
A new LEA is added (new building)
Changes are made in the official state database between July 1 and September 1 of each school year because current year changes must be reflected in the Cycle 1 submission. Therefore, if a change is turned in late, it will NOT be processed until after July 1 the following school year.
The Form must be submitted and approved before the change(s) become effective. It is imperative to return the forms early to avoid problems with scheduling.
A copy of the Form is attached to this memo; however, it can also be found on the APSCN website - http://www.apscn.org. Select the “APSCN” button on the main menu, scroll to “FORMS,” then select “ADE LEA Number Assignment Change.”
Forms are to be mailed or faxed to the address or fax number located on the bottom of the form. Once forms are received and processed, the APPROVED forms will be faxed back to the school district superintendent with an approval date and time stamp.
