Attached is the Corporal Punishment form required by state and federal law for data collection. This report provides statistics regarding specific corporal punishment disciplinary incidents by race, gender, reason disciplined, class referred, grade, and classification of the student. This corporal punishment incident reporting form wil be the official and only form accepted by the Department. Please do not add or delete categories on this form. This report is collected for district level only. Send original to the Arkansas Department of Education, making sure you maintain a copy for your records.
