The attached form for a Request for Waiver to Academic Standards for Senior High Eligibility in Competitive Interscholastic Activities must be submitted to the Arkansas Department of Education (ADE) if a school district plans to establish a Supplemental Instruction Program for eligibility purposes. This form shall be used when requesting a waiver from state standards for senior high eligibility for student participation in competitive interscholastic activities.

The waiver request form must be accompanied by a school district plan for supplemental instruction in order for the request to be considered. The required Supplemental Instruction Program Plan must follow the appropriate format also attached to this memorandum. Each program plan submitted will be reviewed by ADE staff and approved or disapproved based on the requirements for the program as stated in the State Board of Education's Rules and Regulations Establishing the Academic Standards for Student Participation in Competitive Interscholastic Activities. Districts will be notified in writing of the status of their request.

The required Supplemental Instruction Program must occur outside of the regular school day. However, any school district which would like to submit an alternate proposal for providing the Supplemental Instruction Program during the school day may request a waiver from the State Board of Education for this requirement. Such waiver requests will be considered on a school-by-school basis.

The waiver request forms and plans must be received on or before
December 15, 1999, for the program to be available to students for second semester senior high eligibility this school year. Incomplete forms or plans will be automatically disapproved. The next opportunity to request this waiver will be July 15, 2000.

The waiver request forms with the required signatures and the attached program plans are to be sent to:
