IMPORTANT HOME SCHOOL INFORMATION

Attached is a REVISED NOTICE OF INTENT TO HOME SCHOOL form to be used by parents/guardians in declaring their intent to provide home schooling for their children. Please begin utilizing the revised form immediately. There were no changes made to the Waiver form.

SCHOOL DISTRICTS SHOULD FOLLOW THE GUIDELINES LISTED BELOW IN SUBMITTING ALL NOTICE OF INTENT TO HOME SCHOOL FORMS:

SUBMIT TO DEPARTMENT OF EDUCATION (ADE): Please submit a legible copy of each Notice of Intent to Home School (front and back) filed with your district. Please continue to submit the Intents all year long. Do not submit a copy of the Waiver form.

SUBMIT TO COOPERATIVES: Please submit legible copies of 2000-2001 Notice of Intents that have students listed in grades 5, 7, or 10. (copy front and back of Intent) This is to facilitate the required testing process for the 2000-2001 school year. The co-ops will need this information to inform home school parents of the testing dates, times and locations. This information is also necessary for co-ops to secure an adequate number of test administrators.

Parents/guardians listed on the Notice of Intent forms submitted to the co-ops by September 5, 2000 will receive notification regarding the required testing for the 2000-2001 school year.

Due to the short time line between the beginning of the school year and the required testing, it is important that copies of Intents be submitted to the co-ops and ADE as soon as possible. After the testing window is complete, districts no longer need to submit copies to their cooperative. Do not send copies of the Waiver form. If districts have further questions concerning home school testing, please contact the director of your co-op. Pulaski County school districts do not have a cooperative and will conduct their own testing.

School districts should keep the original Notice of Intent to Home School and Waiver forms for their files.
