Attached is a copy of the K-3 Summer Supplemental Instructional Program Student Data Form. Please complete the form and submit to the Early Childhood and Reading Unit by August 31, 1999, or upon completion of 1999 K-3 Summer School Program. Providing services at grades 4-5 is optional; however, if services are provided, please submit data accordingly.
